Kindergarten Registration is going on now!
To sign your
child up for Kindergarten you will need:
- a Birth Certificate –Your child needs to be 5 years old before November 1 ,2007
- Immunization Record (4 polio, 5 DTP, 3 HEPB, 2MMR and varicella)
- Proof of a Physical (exams dated after 3/1/10 also meets the 1st grade physical
exam requirement)
- Dental Exam
- 2 items of Residence Verification (ie: driver's license, utilities
bills, rental/lease agreement, phone bill, etc that has your name on it.)
