Volunteer Application Information:

Sylvan Union School District is happy to announce the return of family visitors and volunteers to our school campuses starting April 4, 2022.  Our schools will begin welcoming parents, families, and volunteers on campus for some activities that support and/or celebrate our students and staff.  Please, check with your school site on specific details of these activities.

REQUIRED:  All visitors and volunteers will sign in through our KeepNTrack system using their driver's license. If a family member doesnt not have a driver's license they may use another form of government identification sucah as a passport, Tribal Identification Card, Us Military Identification, etc.

All visitors and volunteers will complete and sign a "COVID-19 Daily Illness/Health Assessment" prior to being allowed on campus.

 

Below is a link to the Volunteer Application packet you must complete in order to serve as a volunteer in our district. Printed copies are available at your school site as well.  You will need to provide current proof of tuberculosis (TB) clearance and a copy of your driver's license or valid I.D.

Please return the completed application to the school site in which you would like to volunteer. You will then need to bring a copy of the completed volunteer application to the Sylvan Union School District Office (605 Sylvan Avenue) to receive a Live Scan request form (at no cost to the volunteer).
