Medications
Any and all medication, including over the counter drugs (i.e. aspirin, cough drops, cough syrup) must be authorized in writing by both the parent/guardian and physician using the District's Medication Administration Consent form. This form must be updated annually!

Medications that have met the above criteria must be brought to school by the parent or guardian of the student in the original container labeled with current information. Medication is not to be taken to the classroom or kept in any student's backpack.
Please contact the school Health Clerk if you have any questions or concerns about your child's medication.

Emergency Notifications
If you have any changes in your phone number or address, please let us know immediately either by phone or in person. It is very important that we are able to get in touch with you or someone on your child's emergency card in the event of an emergency.
